Below are some of the most frequently asked questions regarding our services.
Please contact us with any other questions you may have.
If you’re currently missing teeth or in need of an implant, please call us to schedule an appointment. No referral is needed.
However, we do accept referrals. Your general dentist may refer you to our office if they find signs of periodontal disease through the course of a checkup or other dental care appointment. We have excellent relationships with our area dentists and work closely with them to provide you with the best possible outcomes and long-term oral health.
You may also decide to schedule a consultation on your own, without a direct referral.
Just give us a call and we'll schedule a consultation.
You should plan on arriving at least 20 minutes early for your first appointment and for all treatment appointments.
There will be new patient paperwork at your first appointment. There will also be consent forms, and getting started with sedation, if applicable.
Dr. Moore respects your time and that of other patients an needs to start your treatment at the appointed time; not have you just arriving at the appointed time.

Please assist us at the time of your initial visit by providing the following information:
- A list of current medications you are taking.
- Dental benefit information / insurance cards (if applicable).
- If you take antibiotic prophylaxis before dental appointments, take them for your exam with Dr. Moore.
Please Note: All patients under 18 must be accompanied by a parent or guardian at the initial consultation appointment.
Please notify the office if you have a medical condition (e.g. artificial heart valves or joints, heart murmurs requiring pre-medication, severe diabetes, or hypertension).
We will complete an in-depth medical and dental health history and a thorough examination during your first visit.
